All sleep tests require a doctor's order. We can help with the process if needed. For PPO and Medicare, your doctor should send the order, progress notes, and patient demos and insurance insurance information by fax to (877) 855-6227 or by email to orders@sleepdr.com. For HMO insurance, your doctor should request Advanced Sleep Medicine Services through the medical group portal. We typically contact you with 1-2 days after receiving a complete order.
